Insurance Certificates
Certificates providing proof of insurance coverage are often requested by the facility where an LLL event is taking place. If you need such a certificate, please send the information indicated below to insurancecertificates@lllusa.org. You should receive feedback in just a few days.
Information needed:
· Name of Group, Area, or Area Network
· Type of event (Series Meeting, conference, WBWC, etc.)
· Date - either standard meeting date (e.g., the 3rd Thursday of the month) or specific event date
· Name and complete address of event location
· Leader contact and email address
